0 Holbrook, NY Holbrook, NY Part-time Full-time Part-time Full-time $22 an hour $22 an hour The Social Work Counselor will provide education and develop independent living skills with foster care children referred by the Department of Social Services in accordance with agency and regulatory requirements to ensure the highest quality of client care.
Responsibilities:
Conduct monthly home visits.
Assist in developing and facilitating Independent Living Skills Workshops Complete a Step Up Independent Living assessment and treatment plan in partnership with client, foster family and caseworker.
Complete and maintain client case records in a timely manner and in compliance with agency and other regulatory standards.
Complete all required reports, correspondence and other paperwork and submit by requested deadlines.
Coordinate service efforts with DSS caseworker regarding client's plan Supervise student intern Provide contacts and supports for the foster families to facilitate the teaching of independent living skills in the home.
Provide aftercare services to all PI program youth Recruit, assess and evaluate potential scholarship recipients Attend conferences, trainings and workshops as appropriate Assume other tasks and responsibilities assigned to support the operations of the program Qualifications and Requirements:
Bachelor's Degree required.
One year of full-time experience working with adolescents.
Valid NYS driver's license Computer literate with MS Office.
Schedule:
Full Time 35 hours per week.
Mon, Tues.
, Wed.
, Fri.
9am-5pm, Thurs.
